U.S. Navy captain and NASA mission director who supervised six Apollo launches, including the ill-fated Apollo 13 mission, during which an oxygen tank exploded midflight. As a gunnery officer on a navy destroyer, he survived an attack by Japanese kamikaze pilots in World War II. Died: Washington, D.C., Feb. 23, 2000
